Summary:
This individual is a clinically competent, registered Nurse Practitioner who will report to the Chief of Medicine,has demonstrated either expertise in the management of patients with gastrointestinal, hepatic and billary disorders, or whose training and/or prior experience indicates appropriate preparation to be trained to deliver care for patients within these disorders. He/She also effectively delivers care to these patients throughout the continuum of care. The candidate's principal duties and responsibilities are to include but not limited to the following: initial history and physical, initial patient consultation to other medical and surgical services, counseling of patients or their personal representatives regarding the patient's disease and health issues. The candidate will order an appropriate work-up to reach a tentative diagnosis and provide medical/therapeutic intervention to assist in patient care. The candidate will also initiate daily inpatient care management on the nursing floors and document daily progress notes. The candidate will assist in preparation of patients for various endoscopic procedures, including pre-procedural history and physicals, procedural consents and peri-procedural orders. With training by qualified staff after hire, the candidate will also have the capacity to place and remove nasogastric tubes, place or remove rectal tubes, only remove or replace percutaneous gastrostomy tubes, and remove colonic decompression tubes in addition to his/her established set of general nursing skills.
Other information:
Gastroenterolgy experience preferred. .Must have knowledge of pharmacology principles, surgical procedures and in-depth anatomy; skill in manual dexterity; ability to access clinical situations and initiate appropriate treatment, interpret laboratory data, respond to emergency situations, coordinate work schedules to achieve maximum productivity and efficiency and ability to recognize and document any pathophysiology. Ability to appropriately function in the electronic medical records environment in both Cerner EMR (hospital) and Athena EMR (office practice).
Responsibilities:
Reaches tentative diagnosis and orders laboratory tests. Initiates daily patient care management on nursing floors, step-down units and documents in daily progress notes. Remove and replace PEG tubes, apply, remove and change dressings and bandages, and perform venous punctures for blood sampling, cultures and IV catheterization. Assists with the interview process of other PAs. Responsible for training and evaluation of these individuals. Attends departmental in-service meetings, conducts in-services and provides education/training to those who will benefit from their expertise. Make rounds on hospitalized patients, direct care as specified by medical staff approved protocols, initiate appropriate referrals, request consultation or referral from nutritional services, speech therapy, physical therapy, cardiac rehab and other health services, initiate , change, and discontinue medication(s) and therapeutic modalities, such as laboratory tests, medications hemodynamic monitoring, treatments, x-ray, EKG, IV fluids and electrolytes, etc., record progress notes-attending physician co-signature required within 24 hours, but prior to discharge, and dictate discharge summaries-attending physician co-signature required UNLESS criteria for discharge are included in a prior order or progress note written by the attending physician that allows discharge by the nurse practitioner. Assist in surgery to include, but not limited to, first assist, deep and simplified tissue closures, application of appliances, and any other action delegated by the surgeon. Performs preoperative assessments, History & Physicals, education on counseling of patients, their Personal Representatives and Caregivers regarding patient's health issues. Initiates preoperative patient preparation to include order sets, consents and other documentation. Provide for the promotion of health and protection from disease by assessing for physiological, psychological and health care system risks associated with the care of complex acute , critical and chronically-ill patients.
